APPOINTMENTS
Although walk-ins are acceptable, to avoid wait times, we recommend scheduled appointments. Same-day appointments (set aside times within the schedule to place appointments for patients requesting to be seen that days are available and are treated as regular appointment visits.

Appointments can be made during our regular office hours.
Monday, Wednesday, Thursday, Friday – 8:00 a.m. – 5:00 p.m.
Tuesday – 9:00 a.m. – 6:00 p.m.
Saturday – 8:00 a.m. – 12:00 p.m.

When making an appointment please consider the following so that we can avoid scheduling delays and complications:
 Call several days in advance when possible.  Describe the nature of your problem to the person making your appointment.  If you must cancel an appointment, please give us as much notice as possible, as this will allow us to see someone who may be in need of an earlier visit.  If you are going to be late, please call the office.

GETTING READY FOR YOUR VISIT
In order for us to provide the best care for you and complete your registration in a timely manner please bring the following items to your visit:  Photo Id  List of current medicines  Insurance card  Copy of medical records from other providers you think may be helpful
If you are uninsured or underinsured and interested in applying for our sliding fee discount, please bring the following items to your visit.
 Proof of Income (if employed): One of the following: > 1040 > W2 > 2 recent pay stubs > Written notarized statement by employer  Proof of Income (if unemployed): One of the following: > Public Assistance check stub/copy > Social Security check or letter of award > Certification Letter from Medical Assistance or Department of Social Services > Written notarized statement from friend or relative with who patient lives (if other forms not available) > Letter of reference from a 501(c)(3) organization, such as a church (if other forms not available)
 Proof of Address: One of the following: > Driver’s license > Any document (envelope) recently addressed to patient such as a utility bill > A written notarized statement by relative or friend with whom patient lives  Proof of Address for Immigrants: One of the following: > Form 1551 > Form 194

PRESCRIPTION RENEWALS
Prescriptions are renewed during normal office hours. In some instances, we may require that patients be seen in our office prior to medication renewal.
Due to the volume load experienced for prescription renewal requests, please make your request prior to running out of your medication. We recommend requesting renewals at least 2 days prior to last dose of medication.

TEST RESULTS
Test results will include lab, radiology and pathology. As test results are available and reviewed by your provider you will be contacted with those results. Please allow three working days before contacting the office for results.
